    目的 观察电针(electroacupuncture,EA)对糖尿病神经痛(diabetic neuropathic pain,DNP)大鼠体质量、空腹血糖、热痛阈、背根神经节(dorsal root ganglion,DRG)瞬时受体电位香草蛋白 4(transient receptor potential vanilloid 4,TRPV4)的影响。 方法 采用向腹腔注入链脲佐菌素( streptozotocin,STZ)构建 DNP 大鼠模型。 第 15 ~ 21 天,在大鼠双侧足三里(ST36)和昆仑(BL60)穴予以每日一次 30 min 的 2 Hz EA 治疗。 检测各组大鼠体质量、空腹血糖和热痛阈。 采用免疫荧光检测大鼠 L4 ~ L6 DRG 中 TRPV4 和神经元(neuron,NeuN)共标的表达。 检测 TRPV4 激动剂 GSK1016790A 对 EA 干预 DNP 大鼠体质量、空腹血糖、热痛阈的影响。 结果 第 7 天后,与正常组相比,模型组体质量明显下降(P<0. 01),空腹血糖明显上升(P<0. 01);第21 天,与模型组相比,模型 + 电针组热痛阈明显提高(P<0. 01);大鼠 L4 ~ L6 DRG 上的 TRPV4 和 NeuN 共标免疫荧光结果显示:模型组的阳性细胞表达较正常组均显著升高(P<0. 01),模型 + 电针组大鼠 L4 ~ L6DRG TRPV4 和 NeuN 共标的阳性细胞表达较模型组均显著下降(P<0. 01);TRPV4 激动剂 GSK1016790A 可翻转 EA 对 DNP 大鼠热痛阈下调作用(P<0. 01)。 结论 EA 能减轻链脲佐菌素诱导的 DNP,其作用机制可能与其抑制 DRG 中 TRPV4 的表达有关。

    Abstract:

    Objective To explore the function of electroacupuncture ( EA) on body mass, fasting blood glucose, heat pain threshold, and transient receptor potential vanilloid 4 (TRPV4) in the dorsal root ganglia (DRG) of rats with diabetic neuropathic pain (DNP). Methods A DNP rat model was formed by intraperitoneally injecting the animals with STZ. From days 15 to 21, bilateral Zusanli and Kunlun points of the DNP rat model were treated with electroacupuncture once daily for 30 min. We then measured their body mass, fasting blood glucose, and heat pain threshold. The co-expression of TRPV4 and NeuN in the rat L4 ~ L6 DRG was detected by immunofluorescence. The effects of the TRPV4 agonist GSK1016790A on body mass, fasting blood glucose, and the heat pain threshold of DNP rats treated with electroacupuncture were detected. Results After the 7th day, body mass was significantly decreased (P<0. 01) and fasting glucose was significantly increased (P<0. 01) in the model group compared with the normal group. After the 21st day, compared with the model group, heat pain threshold of the model + electroacupuncture group was significantly higher (P<0. 01); the Results of co-expression of TRPV4 and NeuN immunofluorescence on rat L4 ~ L6 DRG showed that: the expression of positive cells in the model group was significantly higher (P<0. 01) than that in the normal group, the co-expression of TRPV4 and NeuN positive cells in L4 ~ L6 DRG of rats in the model + electroacupuncture group was significantly lower ( P<0. 01) than that in the model group. The TRPV4 agonist GSK1016790A can reverse the downregulation of thermal pain threshold induced by electroacupuncture in DNP rats (P<0. 01). Conclusions Electroacupuncture alleviated the DNP induced by STZ, and its mechanism may involve the inhibition of TRPV4 protein expression in the DRG.
