    缺血性脑卒中(IS)的疗效长期受限于狭窄的时间窗、再灌注损伤(CIRI)和神经保护剂缺乏的困境。缺氧诱导因子-1α(HIF-1α)作为整合缺氧应答的核心转录因子,通过调控神经炎症、能量代谢、铁死亡、自噬与血管新生关键病理环节,构成了贯穿IS全程的动态调控网络,为破解上述困境提供了新的靶点。本文系统综述了近三年来中药通过调控HIF-1α通路干预IS的研究进展,结果表明:中药及其复方能够通过多靶点、时序性与双向调节的方式,精准干预该网络,从而实现对IS从早期损伤抑制到后期修复促进的全病程保护,为IS的防治开辟了独具特色的视角。

    Abstract:

    []The long-term efficacy of ischemic stroke (IS) has been constrained by the narrow therapeutic time window, cerebral ischemia-reperfusion injury (CIRI), and the lack of effective neuroprotective agents. Hypoxia-inducible factor-1α (HIF-1α), as a core transcription factor integrating the hypoxic response, regulates key pathological processes such as neuroinflammation, energy metabolism, ferroptosis, autophagy, and angiogenesis, forming a dynamic regulatory network that spans the entire course of IS. This offers a novel target for addressing the aforementioned challenges. This article systematically reviews the research progress over the past three years on traditional Chinese medicine (TCM) interventions in IS through the regulation of the HIF-1α pathway. The results indicate that TCM and its compound formulations can precisely modulate this network in a multi-target, temporally specific, and bidirectional manner, thereby providing comprehensive protection throughout the entire course of IS—from early injury suppression to late-stage repair and promotion. This opens up a unique perspective for the prevention and treatment of IS.
