    摘要:

    目的 通过观察乌司他丁(UTI)对大潮气量机械通气大鼠肺组织 Clara 细胞分泌蛋白(CC16)表达的影响,探讨 CC16 在机械通气所致肺损伤(VILI)发病中作用以及 UTI 对 VILI 的干预作用。方法 24 只雄性 Wistar大鼠随机分为对照组、大潮气量组和 UTI 干预组,观察其肺组织病理学改变,测定肺组织中丙二醛( MDA) 和 BALF中总蛋白( TP)含量,采用逆转录聚合酶链反应(RT-PCR)法检测肺组织 CC16 mRNA 的表达,采用免疫组织化学染色法检测肺组织中 CC16 蛋白表达及 Clara 细胞计数。结果 与对照组比较,大潮气量肺组织 MDA 的含量及 BALF总蛋白含量明显升高(P < 0. 01) ,而肺细支气管上皮细胞 CC16mRNA 及其蛋白表达水平明显降低(P < 0. 01) ;与大潮气量组比较,UTI 干预组大鼠肺组织中 MDA 的 含 量 及 BALF 总蛋白含量明显降低( P < 0. 01) ,而肺细支气管上皮细胞 CC16mRNA 及其蛋白表达水平明显升高(P < 0. 01)。结论 大潮气量机械通气导致肺组织 CC16 mRNA 及其蛋白表达水平降低在 VILI 发病中起重要作用,乌司他丁不但能促进 Clara 细 胞 分 泌 CC16 而抑制肺组织炎症反应,还能抑制脂质过氧化物的产生,对 VILI 有一定保护作用。

    Abstract:

    Objective To explore the role of ulinastatin (UTI) in the treatment course of ventilator-induced lung injury (VILI) by investigating the effect of UTI on expression of Clara cell secretory protein (CC16) in the lung of rats in high tidal volume mechanical ventilation. Methods Twenty-four male Wistar rats were randomly divided into three groups:control group,high tidal volume ventilation group and UTI pretreatment group. The levels of MDA in the lung and total protein in bronchoalveolar lavage fluid ( BALF) were measured. The expression of CC16 mRNA in the lung was detected by reverse transcription polymerase chain reaction (RT-PCR) . The number of Clara cells and synthesis of CC16 were detected by immunohistochemistry. Results Comparing with the control group,the levels of MDA in lung and total proteinin bronchoalveolar lavage fluid were very significantly increased ( P < 0. 01 ) in the high tidal volume ventilation group,and the expression of CC16 was significantly decreased ( P < 0. 01 ) . Comparing with the high tidal volume ventilation group,the levels of MDA in the lung and total protein in bronchoalveolar lavage fluid were significantly decreased (P < 0. 01 ) in the UTI-pretreatment group,and the expression of CC16 mRNA and protein increased significantly. Conclusions CC16 plays an important role in ventilator-induced lung injury (VILI) . The expression of CC16 mRNA and protein are reduced in a VILI rat model. Ulinastatin may effectively prevent the inflammatory process and ventilator-induced lung injury by increasing the level of CC16 and inhibit the lipid peroxidation.
