    目的 比较针刺、温针灸、电针三种不同针灸方法对膝骨关节炎模型白细胞介素-1β(interleukin-1β,IL-1β)、肿瘤坏死因子-α(tumor necrosis factors-α,TNF-α)、转化生长因子-β1(transforming growth factor-β1,TGF-β1)、基质金属蛋白酶-1(matrix metalloproteinase-1,MMP-1)、基质金属蛋白酶-3(matrix metalloproteinase-3,MMP-3)及金属蛋白酶组织抑制剂-1(tissue inhibitor of metalloproteinase-1,TIMP-1)的影响,探讨不同针灸方法治疗膝骨关节炎在关节软骨基质代谢方面的差异。方法 选用新西兰兔30只,随机分为正常组、模型组、针刺组、温针灸组及电针组,每组6只。通过切断前交叉韧带建立实验性膝骨关节炎模型,各治疗组选取“后三里”、“犊鼻”穴,分别施以单纯针刺、温针灸及电针治疗,共治疗20次。治疗结束后采用ELISA法检测各组血清中IL-1β、TNF-α、TGF-β1、MMP-1、MMP-3及TIMP-1水平,并比较组间各指标的差异。结果 与模型组比较,各治疗组血清中IL-1β、TNF-α、TGF-β1、MMP-1水平均明显下降(P < 0.05或P < 0.01),温针灸组MMP-3水平明显升高(P < 0.05);各治疗组间比较,电针组MMP-3水平低于针刺组、温针灸组(P < 0.01),IL-1β、TNF-α、TGF-β1、MMP-1水平各治疗组间比较差异无统计学意义(P > 0.05)。结论 针刺、温针灸、电针均能调节膝骨关节炎模型血清中异常的IL-1β、TNF-α、TGF-β1、MMP-1水平,使软骨退行性变的部分因素得到控制,从而起到延缓软骨退变的作用,其中温针灸的调节作用较为显著。

    Abstract:

    Objective To investigate the influence of acupuncture,warm-needle acupuncture, electro-acupuncture on cartilage metabolism-related factors of the experimental knee osteoarthritis model, and explore the similarities and differences of mechanism in different kinds of acupuncture treatment. Methods New Zealand rabbits were equally randomized into control, model,acupuncture, warm-needle acupuncture and electro-acupuncture groups. KOA model was duplicated by anterior cruciate ligament transaction (ACLT). After KOA model was established one week, acupointsof“Dubi”(ST35),“Zusanli”(ST36) were used for treatment with different acupuncture treatmentstwenty times totally. The concentration of IL-1β, TNF-α, TGF-β1,MMP-1,MMP-3 and TIMP-1 in serum were detected by enzyme linked immunosorbent assay(ELISA). Results In comparison with control group, the concentration of IL-1β, TNF-α, TGF-β1,MMP-1 increased significantly in model group(P < 0.05),while MMP-3 reduced significantly(P < 0.05); compared with model group, IL-1β,TNF-α, TGF-β1,MMP-1 reduced significantly in each treatmentgroup(P < 0.05 or P < 0.01),MMP-3 increased significantlyin warm-needle acupuncture group(P < 0.05); comparison of IL-1β,TNF-α, TGF-β1,MMP-1in serum was not statistically significant (P >0.05) between treatment groups; MMP-3, TIMP-1 were significantly lower in electro-acupuncture group compared with acupuncture and warm-needle acupuncture group (P < 0.05 or P < 0.01).Conclusions Acupuncture,warm-needle acupuncture and electro-acupuncture all can regulate cartilage metabolism-related factors and effectively delay cartilage degeneration,especially warm-needle acupuncture.
