    摘要:

    卵巢储备功能减退(DOR)是一种常见的妇科疾病。 随着晚婚晚育趋势的加剧,其发病率呈上升且年轻化趋势,严重影响女性生育能力及生活质量。 目前西医治疗 DOR 主要依赖激素替代及辅助生殖技术,但存在副作用大、治疗成本高等问题。 中医药在改善 DOR 方面具有多靶点、多途径的优势,能有效调节激素水平、促进卵泡发育,且安全性高,已成为临床重要补充治疗手段。 磷脂酰肌醇 3-激酶/ 蛋白激酶 B(PI3K/Akt)信号通路是调控卵巢颗粒细胞增殖、凋亡、自噬、氧化应激的核心通路,其异常激活或抑制与 DOR 发生发展密切相关。 本文综述了 PI3K/ Akt 通路在 DOR 中的作用机制,包括细胞凋亡、炎症反应、自噬及氧化应激等方面,并总结中药单体(如山奈酚、姜黄素)和复方(如育麟方、毓麟护坤汤)通过调控该通路改善 DOR 的最新研究进展,为中医药精准干预 DOR 提供理论依据。

    Abstract:

    Diminished ovarian reserve (DOR) is a common gynecological condition. The incidence of DOR is increasing in line with the trend of delayed marriage and childbirth, showing a younger age of onset and seriously affecting female fertility and quality of life. Current Western medical treatments for DOR mainly rely on hormone replacement and assisted reproductive technologies, but these are associated with significant side effects and unreliable efficacy. Traditional Chinese medicine ( TCM ) offers advantages in terms of multi-target and multi-pathway regulation, effectively modulating hormone levels, promoting follicular development, and delaying ovarian aging with high safety, making it an important complementary clinical approach. The phosphatidylinositol 3-kinase / protein kinase B ( PI3K/ Akt ) signaling pathway is a core regulator of ovarian granulosa cell proliferation, apoptosis,autophagy, and oxidative stress, and its dysregulation is closely linked to the pathogenesis and progression of DOR.This article reviews the mechanisms of the PI3K/ Akt pathway in DOR, including apoptosis, the inflammatory response, autophagy, and oxidative stress, and summarizes recent research progress on the role of TCM monomers (such as kaempferol and curcumin) and compound formulas (such as Yulin Formula and Yulin Hukun Decoction) in improving DOR by regulating this pathway, providing a theoretical basis for precise TCM interventions in DOR.
