    脑缺血再灌注损伤(CIRI)是缺血性脑卒中治疗过程中自由基介导的继发性脑损伤,线粒体DNA(mtDNA)可作为危险信号激活TLR9、NLRP3炎症小体和cGAS-STING等先天免疫信号通路,继而激发下游炎症反应,诱导细胞死亡,在CIRI的发生发展中起重要作用。中药调控mtDNA释放及相关信号通路可以减轻炎症反应,在CIRI治疗中有明显优势。本文综述了mtDNA在CIRI中的释放及其介导的先天免疫信号通路相关分子机制,以及中药通过此机制发挥抗CIRI作用的相关文献,以期为中药干预CIRI的作用及机制提供参考。

    Abstract:

    Cerebral ischemia-reperfusion injury (CIRI) is a secondary brain injury mediated by free radicals during the treatment of ischemic stroke. Mitochondrial DNA (mtDNA) can act as a danger signal to activate innate immune signaling pathways such as TLR9, NLRP3 inflammasome, and cGAS-STING, thereby triggering downstream inflammatory responses and inducing cell death, playing an important role in the development of CIRI. Traditional Chinese medicine (TCM) can regulate the release of mtDNA and related signaling pathways to alleviate inflammatory responses and has obvious advantages in the treatment of CIRI. This article reviews the release of mtDNA in CIRI and its related molecular mechanisms in mediating innate immune signaling pathways, as well as the relevant literature on the anti-CIRI effects of TCM through this mechanism, in order to provide a reference for the role and mechanism of TCM intervention in CIRI.
