    [] 风湿免疫类疾病作为一类免疫炎性级联反应为主要表现的慢性炎症性和自身免疫性疾病,其发病机制复杂,涉及多种免疫细胞和炎症因子的异常活化。巨噬细胞是调节机体免疫的主要介导细胞,巨噬细胞胞葬是指细胞在胞葬相关基因的调控下,利用自身溶酶体降解清除凋亡细胞、维持免疫稳态和促进炎症消退的过程。最新研究发现,巨噬细胞是维持风湿免疫类疾病机体免疫平衡的主要介导细胞,巨噬细胞胞葬在风湿免疫类疾病中具有重要调控作用,参与机体免疫稳态的维持,影响疾病的发生与发展。因此,巨噬细胞胞葬在风湿免疫类疾病中的作用机制已成为该领域的研究热点。近年来,靶向胞葬通路的药物研究进展迅速,提示其可能成为治疗风湿免疫类疾病的新方向。中医药在风湿免疫类疾病的综合治疗中应用广泛,疗效确切。研究发现,中药活性成分如青藤碱、黄芪多糖、公藤内酯醇、小檗碱可通过调控相关信号通路,上调胞葬相关蛋白Mer酪氨酸激酶(MerTK)、生长停滞特异性蛋白6(Gas6)、清道夫受体A(SR-A)、微管相关蛋白1轻链3(LC3)等的表达,从而激活巨噬细胞胞葬作用,调节免疫微环境、炎症反应、血管生成、上皮间质转化、内皮-间充质转化等机制,最终延缓类风湿关节炎、系统性红斑狼疮、系统性硬化症等风湿免疫疾病的进展。现对近年来关于中药活性成分调控巨噬细胞胞葬治疗风湿免疫疾病的研究成果进行总结归纳,以期为临床应用提供新的思路和参考。

    Abstract:

    Rheumatoid immune diseases, as a common type of chronic inflammatory and autoimmune disease, have a complex pathogenesis involving abnormal activation of multiple immune cells and inflammatory factors. Macrophages are the main mediators of immune regulation in the body. Macrophage phagocytosis refers to the process in which cells use their own lysosomes to degrade and clear apoptotic cells, maintain immune homeostasis, and promote inflammation resolution under the regulation of phagocytic related genes. In recent years, studies have found that macrophages are the main mediator cells in maintaining the immune balance of rheumatic immune diseases. Macrophage phagocytosis plays an important regulatory role in rheumatic immune diseases, participating in the maintenance of immune homeostasis and affecting the occurrence and development of diseases. Therefore, the mechanism of macrophage cell burial in rheumatic and immune diseases has become a research hotspot in this field. Meanwhile, the rapid progress in drug research targeting the cellular burial pathway suggests that it may become a new direction for the treatment of rheumatic and immune related diseases. Traditional Chinese medicine is widely used in the comprehensive treatment of rheumatic and immune diseases, with definite therapeutic effects. Various active ingredients of traditional Chinese medicine, such as sinomenine (SIN), astragalus polysaccharide (APS), triptolide (TPL), berberine (BBR), curcumin (Cur), triptolide (TPL), tanshinone IIA, etc., can upregulate the expression of cell burial related proteins Mer tyrosine kinase (MerTK), growth arrest specific protein 6 (Gas6), scavenger receptor A (SR-A), microtubule associated protein 1 light chain 3 (LC3), etc. by regulating relevant signaling pathways, thereby activating macrophage cell burial and exerting immune regulation and anti-inflammatory effects. It analyzes the relevant signaling pathways, regulatory factors, and their characteristics of regulating macrophage phagocytosis, in order to clarify the mechanism of action of active ingredients of traditional Chinese medicine in activating macrophage cell phagocytosis for the treatment of rheumatic and immune diseases, and provide ideas and references for clinical applications.
