    PTEN诱导假定激酶1/泛素蛋白连接酶(PINK1/Parkin)信号通路通过调控线粒体自噬维持线粒体质量稳态中发挥关键作用,其功能障碍与阿尔茨海默病(AD)的发生、发展及潜在的治疗策略密切相关。本文梳理了近年来PINK1/Parkin信号通路在AD中的作用机制及中药复方及活性成分干预调节该通路的研究进展。研究表明,PINK1/Parkin信号通路功能障碍可导致受损线粒体堆积,进而加剧β淀粉样蛋白(Aβ)斑块沉积、Tau蛋白过度磷酸化、神经炎症和氧化应激,形成病理恶性循环,驱动AD的病理进程。在治疗方面,中药(中药复方及活性成分)可通过双向动态调节PINK1/Parkin通路:一方面激活该通路,促进受损线粒体清除,改善线粒体功能;另一方面抑制其过度激活,维持自噬稳态,防止神经元过度死亡。此外,中药还能通过该通路减轻氧化应激、抑制神经炎症、干预Aβ与Tau蛋白病理,并激活SIRT1/3-FOXO3、AMPK等上游调控节点,形成多靶点、整体调节的网络效应。本综述为深入理解AD病理机制及探索基于PINK1/Parkin通路的中药治疗策略提供了重要参考。

    Abstract:

    The PTEN-induced putative kinase 1/E3 ubiquitin ligase (PINK1/Parkin) signaling pathway plays a critical role in maintaining mitochondrial quality homeostasis by regulating mitophagy. Dysfunction of this pathway is closely associated with the pathogenesis, progression, and potential therapeutic strategies of Alzheimer's disease (AD). This review summarizes recent advances in understanding the mechanisms of the PINK1/Parkin pathway in AD, as well as the progress in modulating this pathway through Traditional Chinese Medicine (TCM) compounds and their active ingredients.Studies indicate that impaired PINK1/Parkin signaling leads to the accumulation of damaged mitochondria, thereby exacerbating β-amyloid (Aβ) plaque deposition, Tau protein hyperphosphorylation, neuroinflammation, and oxidative stress. This creates a vicious pathological cycle that drives the progression of AD. In terms of therapeutic intervention, TCM can dynamically and bidirectionally modulate the PINK1/Parkin pathway: on one hand, activating it to promote the clearance of damaged mitochondria and improve mitochondrial function; on the other hand, inhibiting its overactivation to maintain autophagic homeostasis and prevent excessive neuronal death. Furthermore, TCM can alleviate oxidative stress, suppress neuroinflammation, intervene in Aβ and Tau pathologies, and activate upstream regulators such as SIRT1/3-FOXO3 and AMPK, thereby forming a multi-target, holistic regulatory network.This review provides an important reference for a deeper understanding of the pathological mechanisms of AD and for exploring TCM-based therapeutic strategies targeting the PINK1/Parkin pathway.
