    摘要:

    消化道肿瘤作为全球范围内的主要致死性疾病之一,其发病率与死亡率呈逐年攀升态势,防控形势严峻。当前西医治疗虽能在一定程度上抑制瘤体增殖、缓解临床症状,但仍面临术后复发转移风险高、药物不良反应明显及耐药现象频发等诸多挑战,致使整体疗效未尽理想。因此,探寻兼具高效性与安全性的新型治疗策略已成为临床迫切需求。Ras同源基因家族成员A(Ras Homolog Family Member A,RhoA)/Rho关联卷曲螺旋蛋白激酶(Rho-associatedcoiled-coil containing protein kinase,ROCK)信号通路在消化道肿瘤细胞的增殖、分化、迁移及侵袭等病理过程中扮演核心调控角色,深度参与肿瘤的发生与发展,被认为是抗消化道肿瘤治疗的重要靶点。中医药凭借多途径、多机制、多通路的协同作用特点,在抑制肿瘤术后复发转移、减轻放化疗不良反应、提升患者生存质量等方面展现出独特优势,已成为消化道肿瘤综合治疗体系中的关键组成部分。研究表明,萜类、生物碱类、黄酮类、多酚类、醌类及吲哚类等中药活性成分,以及复方斑蝥胶囊、鳖甲煎丸等复方,可通过干预RhoA/ROCK信号通路及其上下游网络,诱导消化道肿瘤细胞凋亡、抑制上皮间质转化进程、阻断血管生成拟态形成、逆转肿瘤耐药性、增强抗肿瘤免疫应答及阻滞细胞周期等,来发挥明确的抗消化道肿瘤效应。本文全面梳理和总结了近年来中医药抗消化道肿瘤的相关研究成果,聚焦中医药调控RhoA/ROCK信号通路治疗消化道肿瘤的作用机制展开论述,以期为临床抗消化道肿瘤新药的研发提供参考与思路。

    Abstract:

    As one of the major lethal diseases in the world, the incidence rate and mortality of gastrointestinal tumors are increasing year by year, and the situation of prevention and control is grim. Although current Western medicine treatment can to some extent inhibit tumor proliferation and alleviate clinical symptoms, it still faces many challenges such as high risk of postoperative recurrence and metastasis, significant adverse drug reactions, and frequent drug resistance, resulting in unsatisfactory overall efficacy. Therefore, exploring new treatment strategies that are both efficient and safe has become an urgent clinical need. The Ras Homologg Family Member A (RhoA)/Rho associated coiled coil containing protein kinase (ROCK) signaling pathway plays a central regulatory role in the proliferation, differentiation, migration, and invasion of gastrointestinal tumor cells, and is deeply involved in the occurrence and development of tumors. It is considered an important target for anti gastrointestinal tumor therapy. Traditional Chinese medicine, with its synergistic effects of multiple pathways, mechanisms, and pathways, has demonstrated unique advantages in inhibiting tumor recurrence and metastasis after surgery, reducing adverse reactions to radiotherapy and chemotherapy, and improving patients" quality of life. It has become a key component of the comprehensive treatment system for gastrointestinal tumors. Research has shown that active ingredients in traditional Chinese medicine, such as terpenes, alkaloids, flavonoids, polyphenols, quinones, and indoles, as well as compound formulas such as Bufalin Capsules and Biejiajian Pills, can induce apoptosis of digestive tract tumor cells, inhibit epithelial mesenchymal transition, block vascular mimicry formation, reverse tumor drug resistance, enhance anti-tumor immune response, and block cell cycle by intervening in the RhoA/ROCK signaling pathway and its upstream and downstream networks, thereby exerting clear anti gastrointestinal tumor effects. This article comprehensively reviews and summarizes the relevant research results of traditional Chinese medicine in the treatment of digestive tract tumors in recent years, focusing on the mechanism of traditional Chinese medicine regulating the RhoA/ROCK signaling pathway for the treatment of digestive tract tumors, in order to provide reference and ideas for the development of new drugs for clinical anti digestive tract tumors.
