Abstract:Gouty nephropathy is the most common microvascular complication of hyperuricemia and also an independent risk factor for the progression of chronic kidney disease to end-stage renal disease. Its high disability rate and heavy medical burden have become a global public health challenge. At present, western medicine treatment is centered on uric acid lowering and anti-inflammation, but long-term use of these medications may lead to adverse effects such as liver injury and bone marrow suppression, and it is difficult to block the process of renal interstitial fibrosis mediated by urate crystals. Traditional Chinese medicine (TCM) possesses unique advantages of "holistic regulation and multi-target intervention" in the prevention and treatment of chronic diseases. Based on the core theory of "Treating water must first treat qi" proposed by Zhang Jingyue, this article focuses on Cinnamomi Ramulus, a classic TCM herb with the effects of "warming and unblocking yang-qi, promoting qi movement to induce diuresis". It systematically sorts out its core active components, including volatile oils (such as cinnaldehydum), flavonoids (such as Cinnamomi glycoside), and organic acids (such as cinnamic acid). From the perspectives of regulating the balance of uric acid metabolism, inhibiting inflammatory cascade reactions, improving oxidative stress, resisting renal cell death, delaying renal fibrosis, and regulating the function of the "gut-kidney axis", this article summarizes the core mechanisms of its intervention in gouty nephropathy, and also offers new ideas for the research and development of new TCM drugs and clinical medication for TCM intervention in gouty nephropathy.
