    目的 探讨烙灸干预方法通过OPN、NLRP3相关炎症信号因子对膝骨关节炎(knee osteoarthritis, KOA)病理过程的影响。方法 6月龄新西兰大白兔随机分为5组:对照组,KOA组,治疗组包括三组即烙灸组,MCC950组及联合组(烙灸+MCC950),每组10只。核磁成像检验是否造模成功,观察并评估软骨损伤程度;苏木素-伊红(HE)染色和阿利新蓝染色观察软骨组织病理学变化;透射电镜观察软骨细胞线粒体结构;免疫印迹法(WB)检测软骨组织中骨桥蛋白(osteopontin, OPN)、NOD样受体蛋白3(NOD-like receptor pyrin domain-containing protein 3, NLRP3)、白细胞介素(interleukin, IL)-1β、IL-18和 ? 型胶原蛋白(Collagen ?)、血小板结合蛋白基序的解聚蛋白样金属蛋白酶4(a disintegrin and metalloproteinase with thrombospondin motifs-4, ADAMTS-4)、ADAMTS-5、基质金属蛋白酶3(matrix metalloproteinase-3, MMP-3)、MMP-13蛋白的表达。结果 与对照组相比,KOA组WORMS和OARSI评分升高;与KOA组相比,各治疗组WORMS和OARSI评分均降低(均P<0.05),但组间无差异。电镜观察KOA组软骨细胞中线粒体结构不完整,各治疗组均相对完整。与对照组相比,KOA组软骨组织中OPN表达降低,NLRP3、IL-1β及IL-18表达明显升高;与KOA组对比,治疗组OPN升高,NLRP3、IL-1β及IL-18显著降低(均P<0.05)。联合组的表达IL-1β低于烙灸组,MCC950组IL-18表达高于烙灸组及联合组(均P<0.05)。与KOA组对比,对照组和治疗组的Collagen ? 表达升高,其中烙灸组的Collagen ? 表达显著升高(P<0.05)。与KOA组对比,对照组和治疗组的软骨组织中ADAMTS-4、ADAMTS-5、MMP-3表达显著降低(均P<0.05),治疗组间无变化。对照组和治疗组的MMP-13低于KOA组,对照组和联合组最为显著(均P<0.05)。结论 烙灸可能通过OPN、NLRP3和软骨细胞外基质降解酶的变化、缓解KOA炎症反应与软骨的修复相关。

    Abstract:

    Objective To investigate the effects of branding moxibustion therapy on the pathological progression of knee osteoarthritis (KOA) through modulation of OPN、NLRP3-related inflammatory signaling factors. Methods 6 month old New Zealand white rabbits were randomly divided into five groups (n=10 per group): control group, KOA group and treatment groups: branding moxibustion group, MCC950 group, combination group (branding moxibustion+MCC950). Magnetic resonance imaging (MRI) was performed to verify successful induction of the KOA model and to evaluate cartilage injury. We assessed histopathological changes in cartilage tissues using hematoxylin eosin (HE) staining and Alcian blue staining. We observed ultrastructural changes in chondrocyte mitochondria using transmission electron microscopy. Western blotting was used to detect the protein expression levels of osteopontin (OPN), NOD-like receptor pyrin domain-containing protein 3 (NLRP3), interleukin (IL)-1β, IL-18, collagen I (Collagen I), a disintegrin and metalloproteinase with thrombospondin motifs-4 (ADAMTS-4), ADAMTS-5, matrix metalloproteinase-3 (MMP-3), and MMP-13 in cartilage tissues. Result Compared with the control group, the KOA group showed significantly increased WORMS and OARSI scores. Compared with the KOA group, all treatment groups exhibited significantly reduced WORMS and OARSI scores (all P < 0.05), with no significant differences among the treatment groups. Transmission electron microscopy demonstrated disrupted mitochondrial structures in chondrocytes of the KOA group and relatively intact mitochondrial morphology was observed in all treatment groups. Compared with the control group, OPN expression in cartilage tissues was decreased, the expression levels of NLRP3, IL-1β, and IL-18 were markedly increased in the KOA group. Compared with the KOA group, OPN expression was increased, while NLRP3, IL-1β, and IL-18 expression levels were significantly decreased in the treatment groups (all P < 0.05). IL-1β expression in the combination group was lower than that in the branding moxibustion group, whereas IL-18 expression in the MCC950 group was higher than that in both the branding moxibustion group and the combination group (all P < 0.05). Compared with the KOA group, Collagen I expression was increased in the control and treatment groups, with the most significant increase observed in the branding moxibustion group (P < 0.05). In addition, the expression levels of ADAMTS-4, ADAMTS-5, and MMP-3 in cartilage tissues were significantly lower in the control and treatment groups than in the KOA group (all P < 0.05), with no significant differences among the treatment groups. MMP-13 expression was lower in the control and treatment groups than in the KOA group, with the most pronounced reduction observed in the control and combination groups (all P < 0.05). Conclusion Branding moxibustion therapy may alleviate inflammatory responses and promote cartilage repair in KOA by modulating OPN、NLRP3-related inflammatory signaling factors and cartilage extracellular matrix degradation enzymes.
